4-H Custom Questions Added to Eventbrite

NC 4-H recently requested some 4-H specific questions for all new 4-H events in Extension Eventbrite. These questions are added to all event requests where 4-H Youth Development is selected as the Relevant Program Area. This program area information is used for ERS purposes and to automatically add information to the event when it is created.

The 4 new questions are:

  • Is the participant a registered member of 4-H and enrolled in 4HOnline?
  • The 4-H Liability Waiver is a legal document that is binding on me, my heirs and assigns, and on those who may claim by or through me. I am eighteen years of age or older, and have full capacity to enter into this agreement and do so voluntarily.
  • I understand that my child and I must abide by the 4-H Program Expectations and Parental Permission Rules & Guidelines. I understand that Dismissed Participants may not be eligible for a refund of any fees or expenses.
  • The 4-H Media / Photo Release is a legal document that is binding on me, my heirs and assigns, and on those who may claim by or through me. I am eighteen years of age or older, and have full capacity to enter into this agreement and do so voluntarily.

If you are hosting a 4-H event and you do not need these questions, you can toggle them off by editing your order form in the Order Options > Order Form section of Eventbrite.

You may also choose to add additional to Eventbrite’s standard questions or custom questions if you need the data for your event. Please remember that we should not collect what the university considers sensitive information and store it in Extension Eventbrite. This includes birth dates which are classified as Red data. If you have questions about the storage or classification of data, please refer to Storage Locations for University Data and Determining Sensitivity Levels for Shared Data.

In addition, all Extension Eventbrite events automatically have the USDA demographic reporting questions added. We cannot require these questions to be answered so they should always be listed as optional. The wording of these questions should not be changed.
